AB A prospective study, led in Laennec's hospital from 1985 to 1992,
enabled us to observe 92 cases out of 1248 patients HIV+, about 7% (82
men, 10 women, average age 36.7). These patients were studied clinicaly
by imaging procedure, biology and long-term follow up in order to set up
the signification of these complications. We distinguished the
infectious events requiring a surgical case from the tumerous one.
First, the alithiasic cholecystis (15) must be mentioned, more often by
CMV, crypto and microsporidiosis, frequently with angiocholitis (12),
the sclerosing cholangitis (10) and ano-rectal sepsis. Six acute
pancreatitis arised. Out of the tumors note should be taken of the
visceral Kaposi's tumors and the N.H. Lymphomas which give mass effect
and obstruction, sligth or dramatic hemorrage, or perforation with
peritonitis. The common carcinomas have an abnormal swiftness of growth.
These complications chiefly happen in the C stage, among other events
and their numerous treatments. Thereafter, the diagnosis and the
treatments are deeply hindered. In the short term, the surgical
treatment may obtain some success, but that does not modify the
inexorable course of the disease. 65 patients were operated, and 12 not,
in terminal status. 62 died, despite a post-surgical improvement, 15 are
in threatened condition, 10 in remission and 5 post in sight. The
average survival time after cholecystectomy (15) is 170 days, after
surgical procedure for Kaposi's tumors (10) is 10.5 months, and for
lymphomas (12) is 5 months. The surgical complications often herald the
getting worse period and have a worrying signification.
